Sports Compression Became Fashion. Who is Advancing the Therapy?

The sports compression industry has a paradox.
It has never been more visible and it has never been harder to see where the therapy itself is going.
Compression has become fashion. It has become lifestyle. It is now a familiar part of sportswear, recovery routines and everyday wardrobes.
That should be celebrated.
But it raises an uncomfortable question for an industry that likes to call itself innovative:
Have we become better at selling compression than advancing it?
I believe we have.
And I believe the next major opportunity will not come from another fabric, colour or garment marketed as “recovery”. It will come from treating compression as technology rather than clothing.
That is a very different proposition.
We turned therapy into a category.
The commercialisation of compression has been extraordinarily successful.
What was once specialist equipment became mainstream sportswear. The visual language changed. Compression became sleek, aspirational and easy to understand. Consumers did not need to know much about physiology.
Put it on.
It feels supportive. It looks athletic. It is associated with performance and recovery.
The category essentially sold itself.
But something happened along the way: the word compression began doing too much work.
Performance compression. Recovery compression. Travel compression. Everyday compression. Medical compression. Wellness compression.
These are not necessarily the same intervention. Yet commercially, they increasingly sit under one umbrella.
That is convenient.
Scientifically, it is problematic.
The industry has an apparel problem.
The biggest limitation in compression today may not be the technology. It may be the way we think about it.
Compression is still largely developed through the lens of apparel.
We begin with the garment. We choose the fabric. We optimise comfort. We determine the size range. Then we ask what benefits we can associate with it.
I think the order should be reversed.
Start with the person. Start with the objective. Start with the physiology.
Then determine what intervention is required and only then design the product capable of delivering it.
That sounds obvious. But it represents a profound change in how the industry operates.
A garment is not a therapy
A garment is an object.
Therapy is an intervention.
One can be evaluated aesthetically. The other needs to be evaluated by what it does.
The compression market sometimes treats these concepts as interchangeable.
They are not.
A garment can be beautifully engineered and incredibly comfortable without delivering a meaningful therapeutic intervention. Equally, a technically effective intervention will ultimately fail commercially if nobody wants to wear it.
The challenge, therefore, is not choosing between performance and usability.
It is combining them.
The best compression technology should disappear into the experience of the person wearing it. The complexity should be behind the product not on the body.
Stop asking: “Does compression work?”
This may be an unpopular view, but I think one of the least useful questions in compression research is:
Does compression work?
The answer depends on what we mean by compression, who is wearing it, what they are doing, what outcome we are measuring and when the intervention is applied.
Research into sports compression has reported potential effects across areas such as perceived soreness, recovery, blood flow and neuromuscular responses, while findings for performance enhancement have been much less consistent.
That should not be interpreted as a failure of compression.
It should be interpreted as a failure of the question.
The better question is:
For whom does which compression intervention work, under what circumstances?
That is a much more interesting problem.
And solving it requires us to move beyond the generic garment.
The future is not one-size-fits-all
One of the strange contradictions in modern healthcare is that we increasingly talk about personalised medicine while compression remains heavily standardised.
We accept that individuals respond differently to drugs. Rehabilitation programmes are adapted. Training plans reflect the athlete. Nutrition can be personalised.
But compression?
Choose a size.
Choose a level.
Put it on.
The assumption is that the garment will somehow accommodate the complexity of the human body.
Why?
The body is not a standardised cylinder.
People have different anatomy, proportions, tissue characteristics, movement patterns and physiological needs.
The next generation of compression needs to move beyond sizing and towards personalisation.
And that does not simply mean making garments in more sizes. It means designing the intervention around the individual.
Personalisation is where compression gets interesting.
Imagine a future where the question is no longer:
Which compression garment should I buy?
Instead:
What intervention is appropriate for me?
That intervention could be determined by anatomy, activity, recovery objective, clinical need, duration and other individual characteristics.
Technology can increasingly support that model.
Body scanning can capture dimensions that traditional sizing systems cannot. Digital modelling can help predict how a garment interacts with an individual. Advanced manufacturing can make greater personalisation economically viable. Measurement technologies can help us understand what is actually happening between the garment and the body.
Data can move us from generic assumptions towards individual evidence.
None of this requires compression to become a piece of wearable electronics covered in sensors.
In fact, the best technology may be almost invisible.
The biggest innovation may be the one consumers never see
The apparel industry is trained to think visually.
Innovation needs to be visible: a new fabric, panel, seam, construction or silhouette.
Healthcare technology often works differently.
The most important innovation may be invisible.
A better algorithm.
A more accurate measurement.
A more sophisticated fitting process.
A different manufacturing technique.
A new way of validating an intervention.
A better understanding of individual response.
Compression should embrace that model.
The next breakthrough may not look revolutionary.
It may simply work better.
Compression is not simply about wearing a tighter garment.
It is about applying an appropriate level of pressure to achieve a desired physiological outcome.
Different athletes have different anatomies.
Different sports create different demands.
Different stages of training require different recovery strategies.
Yet most sports compression products continue to rely on standard sizing and fixed compression levels.
The conversation has become centred on garments rather than therapy.
A mature industry should be comfortable with complexity. We should be able to tell consumers what we know, what we do not know and what we are trying to discover.
That is not weak marketing.
It is credible innovation.
The real opportunity is the gap between laboratory and life.
There is a fascinating disconnect in compression.
We can measure things in controlled research environments. We can specify materials, develop prototypes, test pressure and study physiological responses.
Then we give someone a garment and ask them to wear it for hours, move around, sweat, wash it and use it repeatedly.
Real life is messy.
That gap matters.
The compression intervention people experience outside the laboratory is not necessarily identical to the intervention we designed inside it.
This is where engineering becomes critical.
The future of compression will require us to understand not just how a product performs when it is new, but how it behaves as part of real life.
Because that is where therapy happens.
Not in a product photograph.
Not on a mannequin.
Not in a marketing presentation.
On a person.
Compression needs to become a platform
I believe the most successful compression companies of the future will think differently about their products.
They will not see a compression garment as the final product.
They will see it as part of a platform connecting the individual
→ the assessment → the intervention → the garment → the measurement → the outcome.
That changes the business model.
It changes product development. It changes research. It changes the role of manufacturing. And it changes what “innovation” means.
Instead of launching another garment every season, the objective becomes improving the intervention itself.
That is a much harder problem.
It is also a much more valuable one.
This is bigger than sport.
Sport has been one of the most visible drivers of compression, but the underlying opportunity is much broader.
The same principles of individualisation, measurement and evidence can apply across recovery, rehabilitation, clinical care and everyday health.
Sport simply gives us an unusually demanding environment in which to develop the technology.
Athletes do not want theoretical performance. They want something that works when the body is under stress.
They move. They sweat. They repeat the activity. They travel. They recover quickly. They demand comfort and performance simultaneously.
That makes sport a powerful proving ground.
But the technology should not stop there.
So who is advancing the therapy?
This is where I believe the industry needs a new kind of company.
Not another apparel brand.
Not simply another medical device manufacturer.
Something between the two.
A company that understands the body, the science, the engineering and the reality of human behaviour.
A company prepared to ask uncomfortable questions about what compression actually delivers.
A company willing to invest in measurement rather than marketing alone.
A company that sees personalisation not as a premium feature, but as the logical destination of the technology.
That is the direction we are taking at ISOBAR.
Our ambition is not to make compression more fashionable. The fashion industry has already solved that problem.
Our ambition is to help make compression more intelligent.
The industry should compete on outcomes
For too long, compression products have competed on specifications.
Fabric composition. Compression class. Construction. Breathability. Seam technology. Fit. Price.
These are useful characteristics.
But they are not the ultimate outcome.
The more important question is:
What difference does the intervention make for the person wearing it?
That is where the industry should ultimately compete.
Not who has the most advanced fabric.
Not who has the most recognisable logo.
Not who can make compression look the most fashionable.
But who can demonstrate that their approach delivers a better intervention for a specific person and purpose.
That requires evidence.
It requires humility.
And it requires a willingness to challenge some of the assumptions that have become embedded in the category.
Fashion opened the door. Technology must take us further.
I am not nostalgic for the old world of compression.
I do not want compression pushed back into hospitals and specialist clinics.
Quite the opposite.
The fact that millions of people are now comfortable wearing compression is an extraordinary platform for the next stage of innovation.
Fashion opened the door.
Sport pushed the category forward.
Consumer adoption created scale.
Now science and technology need to take the next step.
We need to move from compression as a garment to compression as an intervention.
From standardisation to personalisation.
From claims to evidence.
From specifications to outcomes.
From products to systems.
And from asking whether compression works to understanding when, how and for whom it works best.
That is the opportunity in front of us.
Sports compression became fashion.
I think that was necessary.
But it is not the end of the story.
The next chapter should be about advancing the therapy.
